I grant Christ Church Birmingham, Christ Kids Children's Ministry, and/or Student Ministries permission to publish my child's photo on the website, brochures, and/or any social media accounts for promotional purposes. I understand that if I wish to change this decision, I must do so in writing.

By clicking "I Agree," the participant, parent, or legal guardian of the participant hereby releases, acquits, and forever discharges Christ Church Birmingham of Birmingham, AL, and its employees from all suits, damages, claims, proceedings, demands, and liability for any injury, harm, or damage that the participant may incur while participating in any activities with Christ Church Birmingham and its ministries.

It is further agreed that the participant and/or the participant’s parent or legal guardian shall indemnify and hold harmless Christ Church Birmingham and its employees from all claims of any nature whatsoever, whether legal or equitable, that may arise as a result of the participant’s own negligence while participating in any activity herein.

Furthermore, in the event of an accident, if the staff or a representative is unable to contact the parent or guardian, the undersigned parent or legal guardian of the participant hereby grants permission to the staff or representative to administer necessary first aid and/or take the participant to the nearest medical facility for additional treatment.

We need you! Please prayerfully consider serving at VBS. This is an amazing opportunity to invest in the faith development of children from our church and community. There are many ways to serve at VBS, from group leaders to decorations. You will be trained, encouraged, and equipped to serve in the role you choose.

There is an optional $10 donation fee (with a $20 max per family) to help offset VBS costs, including t-shirts, supplies, and snacks. This fee is completely optional, as we want every child to have the opportunity to attend VBS. If you are a VBS volunteer, please select the volunteer payment option to bypass this section.
